store as usual, only once you click on "check out" and after
you log in you will be sent to your order page.
- Simply print this form out and fax it to 410-684-2106.
Please note on your printed order whether you will be paying by mail in
check (orders will not be shipped
until your check is received and cleared, this usually takes 3 business days
after receiving it) or credit card.
If paying by credit card please write your credit card number on your order
along with the expiration date. Please note: if the name and address you
used during setting up your account is different from you credit card
billing name and address please be sure to write the correct ones on your
order.
